Would sliding genioplasty achieve my desired look? (Photo)

An opening intraoral genioplasty can achieve in a single procedure 10 to 12mm of vertical opening. That is close to what you are simulating  but probably not quite as much. While some horizontal advancement can be achieved at the same time as the vertical opening it wold not be as much s your are imaging. Ultimately not achieve the large amount of chin change you have imaged will require a two-stage chin procedure.

Thank you for your questions and photos. I am sorry that the chin implant did not give you your desired results. I have had several patients come to be for a sliding genioplasty after they are unhappy with an implant. Often times I will remove the implant and move the natural bone, however, in some cases I will keep the implant in order to obtain even more projection. Realistically you can advance the chin an additional 12mm or so horizontally and if you advance more than 6mm vertically often you will need some type of bone grafting material in order to fill the gap between the bones. This can be done safely, and should be thoroughly discussed with your board certified plastic surgeon. I would also suggest asking them to create a computer imaging to show what they see as a realistic outcome from your surgery. I do believe you would benefit both from chin advancement and vertical lengthening.
