POST OP - Bilateral ptosis repair and elevated eyebrow

Question

I recently had bilateral ptosis repair and prior to the repair, my more severely affected eye had an elevated eyebrow from constant raising and straining. I actually have more noticable wrinkles on my forehead on that side. Will this correct itself at all if it has been like this for years? My surgeon said it might and if not, she suggests botox to help make my eyebrows appear more even. Is the likelihood of this self- correcting in time possible or do I need to save up for some botox?

These photographs suggest that you still have very significant upper eyelid ptosis and compensatory eyebrow elevation that is still stronger on the side with more ptosis. I would guess that your ptosis surgery was performed by Müeller's muscle conjunctival resection ptosis surgery. This is by far the most popular form of ptosis surgery because is anatomically the more simple surgery. Regrettably, in eyelids with a significant component of a central levator disinsertion, the surgery in my opinion is often ineffective because the tendon is out of position and is ineffectively plicated by the surgery. These are eyelid that need anterior levator resection ptosis surgery. I would recommend formal crease lowering and fold reconstruction. With a sufficiently strong ptosis repair, the forehead will relax in most cases. I would recommend healing a full six months before having revisional ptosis surgery. You certainly can have a consultation before that. Avoid the Botox injections. They will be unsatisfactory in my opinion.
