Hi, I'm someone with an undergrown nasomaxillary complex, leaving a lot of my soft tissue features sagging, and looking unalert or neotenous to me. I'm curious about how realistic it is to expect total orbital implants atop the zygomatic arch and over the supraorbital ridge to enhance the appearance of the eye shape and make my eye area more masculine or alluring. I would also be seeking out paranasal implants to give the impression of an overall forward grown face. thank you

A higher maxillary vestibule or larger lips does not make it more difficult to place cheek implants intraorally. Your anatomy is not particularly uncommon in this type of facial surgery.
There will be immediate change after removal of implants, have no doubt; capsule around implant is very fine 6 months after surgery and for that reason you will not see much change when swelling goes away. Did you think about smaller implants? Good luck.
Like any implant placement through the lower eyelids management of lip position is an important part of it. At the least a protective lateral canthopexy is needed. But if the goal is to eliminate the scleral show a lateral canthoplasty with spacer grafts are needed.
