I am 41 years old. I have enlarged pores on my nose which are difficult to keep clean and are cosmetically bothering me. I have had clear and brilliant x 3 treatments in 2019. I have had Morpheus 8 x 1 treatment 3 months ago which seems like it made things worse. I apply retin A 0.05% every night for over 12 months without much improvement. Also using salicyclic 2% every other day, 15% salicyclic every other week.
Answer: Shrinking Large Nasal Pores First, let me say that I am not at all surprised by the waste of time and money with the so-called "energy-based" treatments. For the most part these relatively expensive, faddish "bells and whistles" have woefully little hard science and a whole lot of marketing hype to support their use. I have been cautioning my regular readers about this for years now. Sorry. From what I can see, by far the simplest approach would be a very thorough manual cleaning of these large and deeply impacted pores. Once this has bee done, the nose will show an immediate and dramatic improvement. For maintenance thereafter, a series of three or four, in-office, high concentration salicylic peels of the nose can be performed every two to four weeks and complemented by the regular at-home use of a prescription strength topical retinoid and topical salicylic acid preparation would help to prevent and minimize their recurrence. Make sure to consult with a board certified cosmetic dermatologist and not just any venue hawking the latest, expensive fad toy it has just purchased. Hope this helps. Best of luck.

Answer: CO2 laser paired with Melapads to improve results Fractional CO2 laser is an excellent tool for skin rejuvenation in our center in Santa Monica. We use the fractional multi-depth ablative approach to help resurface the skin, recontour acne scars, improve incisional plastic surgery scars, and reduce surface irregularities from skin tone to dark pigmentation. Our approach utilizes a multi-depth approach that targets melanocytes in the epidermis and collagen fibroblasts in the dermis. This approach can be combined with a TCA Peel in a process known as a sapphire peel. Fractional laser is excellent for acne scars and contour scars in the skin. Patients are advised to undergo skin protocols with Melarase creams before and after treatment to help reduce pigmentation. Melarase AM, Melarase PM, and Melapads can seriously improve surface sun damage and UV related pigmentation. After the laser, we expect redness for a few weeks as the skin improves. PRP and Hydrafacial enzyme facials can further improve skin quality after fractional co2. Best, Dr. Karamanoukian Realself100 Surgeon

Answer: Best Procedure for Enlarged Nasal Pores? This is a very common question. Enlarged pores are genetic so people with good pores hit the genetic lottery and then there are the rest of us that didn't! As we age, the pore openings seem to get larger since our collagen levels, which decrease 1% per year starting around age 21, are not keeping the openings as tightly closed so they are more obviously noticeable. I think that the retin A is a great idea but as far as procedures go, I think that the Halo laser gives the most improvement. As a member of the Bad Pores Club, I have struggled with this my entire life and I held the office record for worst pores scores on the Visia Complexion Analysis for years. All the other lasers, etc only improved my pore score from 2% to 8%. My 1st Halo laser improved this to 20th% and the 2nd Halo laser done 1 month later further increased my pore score to 50th%. I now do a Halo laser about every 12-18 mos to maintain better looking pores. By the way, the Visia is used to compare your scores with patients of your age who have the same skin type as you, so it gives you a very objective look at how your pores (as well as several other assessments such as wrinkles, superficial/deep sun damage, texture, redness, etc) really are doing for your age and also allows you to follow your level of improvement. I would suggest getting consultation done with a physician who performs Halo laser and also has a Visia device so you can be properly assessed, treated, and followed to watch your improvement. Best wishes, Mary Beth Mudd, M.D.
