Suggestions on lower eyelid surgery revision? (Photo)

I had lower transconjunctival blepharoplasty eyelid surgery and laser peel done 6 weeks ago. I don't see much change with the fat pads and my Dr agreed. She suggested possibility doing lowers eyelid surgery again but doing the incision under eyelash, where she can access fat better. She suggested fillers as well, but I was hoping the surgery would have taken care of the bags more. Thoughts? Is it possible that the surgery cost would be reduced since little change has occurred?

A physical examination is needed to actually know what is going on in your lower eyelid. Sometimes things that appear to be "eyelid bags" are caused by different things. Lower eyelid surgery addresses herniated orbital fat. But the fullness can actually be caused by unveiled orbital rim bone. If your issue was caused by herniated fat, then a properly done transconjunctival surgery should have resolved the issue. To be honest though, you have a complicated lower eyelid with relatively poor orbital rim support. In studying your before and after, you have had a small degree of improvement. Six weeks is very early from this type of surgery. The lower eyelid fat is still inflamed and swollen from surgery. Rather than subject yourself to confusing second opinion consultations right now, I recommend that you let the eyelids heal for a couple more months and see how these eyelids settle down. You may or many not feel compelled to have more work. I think it is a mistake to make a decision on more surgery at just 6 weeks after surgery. Do not get under eye fillers at this time either. Let yourself heal first. At 4 months after surgery, you will have a much better idea of what was accomplished. Do not rush this time frame.

You are WAY too early to consider a revision surgery. What you see could easily be persistent edema / swelling. I would not consider a revision until you are at least 6 months out.

It is too early to assess your final results.  Swelling lasts at least 8 weeks and sometimes longer.  I would not consider revision  yet.  I would wait 4 months on a decision and would not recommend revisionary surgery for at least 6 months. Good luck, Jane

You ask a very difficult question/s. Doing re operation in lower lids by same surgeon, using external approach can be done but why not obtain second opinions from experts in lower eye lid surgery. Also the additional fees incurred. Then discuss with original surgeon options to use her or have surgery with another surgeon and how could the fees be off set. In my opinion external approach ca be done after 6 months from 1 st surgery to allow the laser peeled lower eye lid skin to heal completely. Good luck.
