Question

My left cheek is much larger that my right and makes it so i have a smile line on only one side. It doesn't look bigger when i don't smile though. I think this happened from only chewing on one side. What can I do about this

You have more fullness on the left than the right according to this photo. This is in the buccal fat area. The buccal fat could be adjusted to likely improve this. It's an in office procedure that takes about 30 minutes.

Cheek asymmetry that appears mainly when you smile is usually caused by differences in muscle activity rather than bone structure. In your case, chewing mostly on one side can enlarge the masseter muscle on that side over time, making that cheek look fuller and creating a deeper smile line.

The most effective non-surgical option is Botox (botulinum toxin) injected into the larger side. This relaxes the overactive muscle and gradually slims it down over a few weeks, improving symmetry. Results typically last a few months and can be maintained with repeat treatments.

Another option is to add volume to the smaller side using dermal fillers. This can balance the face either alone or in combination with Botox, depending on whether the issue is more “too much” on one side or “too little” on the other.

It’s also helpful to correct the underlying habit by consciously chewing on both sides to prevent recurrence.

Surgical options are rarely needed unless there is true structural asymmetry present at rest.

A consultation with an experienced injector is important, as treating facial asymmetry requires careful assessment of both muscle movement and volume distribution to achieve a natural, balanced result.
