I've spent loads on fillers which add a full look to my midface and not a projection which is what I need I had adenoid surgery and then the underbite and overcrowding corrected with braces (in a very sloppy manner) as a teen. Bite is oclccluded, overcrowding has returned which will be corrected. Filling the cheeks is only creating weird puffy smile. the entire midface needs pushed forward and lower face needs to be shorter and wider. What are my options? Leaning towards custom implants.

Both these procedures can safely be performed together. Usually the worst part of the recovery is from the removal of the wisdom teeth (more swelling and limited opening). I routinely perform these procedures in my office under Intravenous sedation.
Sorry to hear your issue. If indeed you have a marginal nerve injury you could have it tested for anatomic and or functional integrity even at five months. That should give you some prognostic guide. Recovery is possible but can take up to 12-18 months.
