Hyperpigmentation Under Both Eyes After Blepharoplasty. What Are My Options? (photo)

Question

I had Lower Blepharoplasty and pear fat transfer about 5 months ago. After the operation I have developed dark hyperpigmentation under both eyes. I have tried Retin-A, Skinoren and Elure creams with no results. What would you recommend next? Is IPL or laser possible in this area? I have type 3 middle eastern skin. Thanks!

Removing lower eyelid fat changes how the pigmentation in the lower eyelid appears. It is possible that an appropriate chemical peel might help you. IPL will not work here because the pigment is too deep to be reached by the IPL treatment. For the same reason the skin creams will be ineffective. Lower eyelid skin is thin for this reason, I personally would not use a trichloroacetic acid agent stronger than 35%. However, phenol 89% is an agent that I often use for lower eyelids. A very helpful alternative is to fill in the tear trough with Restylane which can help the dark area look better.

What you are seeing here is likely a combination of true skin pigmentation and residual blood products from the bruising. The blood product pigmentation will improve further with time. You could consider some skin bleaching creams as well. Consult with a dermatologist.

Yes the hyper pigmentation shows in your posted photo. Try IPL but I might suggest a deep peeling either 70% TCA or modified phenol.
