Although you have a very unusual situation -whether it is from birth or trauma it can be improved by the application of standard rhinoplasty techniques in the hands of an experienced surgeon. Specific attention toward the tip with either cartilage repositioning--- cartilage grafting-- or possibly implant placement will give you the best results.In my opinion, it would be not in your best interests to simply try to cover the cleft.
Wow, interesting, and uniquely you! Easiest way (also least expensive and most immediate) is injectable filler. You however, don't just want the vertical cleft filled, because this would result in a horizontally broad nasal tip. You want the tip narrowed and unified with nice aesthetics to match your other beautiful features.A good examination is important to know size and shape of cartilage, and underlying maxillary support. Depending on findings, a well planned and masterfully executed tip rhinoplasty will "fix" the problem. You probably won't miss it, but some friends and family might.
The best long-term solution for fixing the line in between your tips are to realign the cartilages so that there is no space between them. This would entail a rhinoplasty.Your alternative is to use a dermal filler such as Restalyn or Juvederm. This will provide a transient correction of the deformity that you're interested in. This can be done in very small increments and it can be done while you're watching. The other advantage of this approach is that if you do not like what you see, then you can also inject a little enzyme to reverse the dermal filler product. I would definitely go to someone who is experienced in the nonsurgical rhinoplasty.
A tip rhinoplasty would have a far more predictable and long lasting result than would a filler.Find a board certified plastic surgeon who performs hundreds of rhinoplasties and rhinoplasty revisions each year. It appears that you have bifid tip. This is when the right and left lower lateral cartilages that form the tip are slightly separated in the midline. This can be addressed during rhinoplasty and if it is the only area that you are concerned with can be quite predictable to correct. Another alternative is injection of a soft tissue filler such as a hyaluronic acid based product (e.g., Juvederm, Restylane). This is a nonsurgical procedure and these temporary fillers can last upwards of 2 years in the nose - do NOT allow anyone to inject a permanent filler in your nose (e.g., silicone, Artecol). From an aesthetic point of view, this can make the overall size of the tip appear larger and this is something to keep in mind. Additionally, when having injections into the nose, it is important that you have a surgeon that does these frequently in the nose as they can be somewhat more challenging than other areas of the face. You should consult with a board certified plastic surgeon to discuss these options.
The line that you are referring to is a natural cleft between left and right lower lateral cartilages of the nasal tip. To remove this cleft, suture techniques to the lower lateral cartilages will obliterate the cleft itself. In addition cartilage grafts can also be inserted in that area to fill in the gap as well. The particular procedure is determined at the time of the consultation and examination for your tip-plasty. A full rhinoplasty may be required to ensure all the components of the nose are balanced. This is a great question for this forum. The "line" is normal and is a separation between the inner edges of the nose tip cartilages. If the two "wings" of the cartilage are brought closer together, you will not see the lines. Cost is hard to estimate but this would be called a "tip rhinoplasty". Find an experienced board certified plastic surgeon with nose surgery experience. Good luck
