Gynecomastia itself is a benign condition characterized by enlargement of the male breast. There are several causes to this, and should certainly be ruled out. In your photos, it does appear that you have some increased size of the tissue within your breast/chest area.A surgical procedure for this condition is certainly your decision and a matter of how concerned you are about the appearance of this area. I would recommend consulting with a plastic surgeon well-versed in the management of gynecomastia who will examine and assist you in deciding if a gynecomastia procedure to ameliorate your concerns would be the right thing for you. Given that there are no concerning abnormalities on examination and it is simply gynecomastia, your degree of gynecomastia could likely be treated with liposuction to the area alone. My feeling is that you do you have some excess tissue on your chest and the nipple is larger than would be expected. Thus you probably have "gynecomastia" - the development of female appearing breasts in a man. If you are bothered by the appearance, then visit a board certified plastic surgeon.
It looks like there are some areas of over-resection. I agree with your doctor that fat grafting to these areas is the best treatment and it may very well take several sessions. Also, any tethered scars may need release and revision. This is a tough problem and correcting it...
