In Nov. 2013, I had a mastopexy and a breast augmentation. I have 340cc (silicone, sub pec) and the results are not what I expected and my breast are still sagging. If I go up to 500cc, will that improve my sagging or will it be worse? Or does a revision to the lift need to be done at the same time?
Answer: Avoid large implants The first decision you have to make is whether you are satisfied with the volume of your breasts. If you want a larger cup size, you will need larger implants. If you are satisfied with your cup size, you do not need larger implants. Next, you need a breast lift. You have undergone a traditional vertical scar technique, which resulted in an inadequate lift. I recommend a new technique called Implant Exchange with Mini Ultimate Breast LiftTM. Using only a circumareola incision it is possible to reshape your breast tissue creating upper pole fullness, elevate them higher on the chest wall and more medial to increase your cleavage. Aligning the areola, breast tissue and implant over the bony prominence of the chest wall maximizes anterior projection with a minimal size implant. Small round textured silicone gel implants placed retro-pectoral look and feel more natural, are more stable, less likely to ripple or have complications needing revision. Implant profile is irrelevant in the retro-pectoral position since the muscle compresses it. The worst thing you can possibly do is add larger implants. Best Wishes,Gary Horndeski, M.D.

Answer: Revision after augmentation-mastopexy will improve symmetry Every situation is different and there is no way to offer much useful advice without an in-person exam, but here are a few things to consider: In general, larger implants make sagging worse in the long run, not better; implants need to be symmetrically placed before assessing nipple position, and your implants do not appear to be at the same level; and your implants also appear to be appropriate in size for the base diameter of your breasts so upsizing is probably not the best idea.

Answer: Breast lift and implant revision surgery can help sagging breasts Thank you for your questions and photos. It appears as though going up to 500cc implants will prevent you from needing a lift, but I'd have to see you in consultation to give you an exact answer. It also looks like the right implant needs to be lowered a little bit. Visit the website listed below for more information, including before and after photos.Kind regards,Dr. SchreiberBaltimore Plastic Surgeon

Answer: Will going to a bigger implant improve my sagging or make it worse? Thank you for the question and pictures. Unfortunately, online consultants will not be able to provide you with specific enough advice to be truly helpful. Keep in mind, that the issues at hand may be more than simple breast implant volume issues; adjustment of the breast implant pockets and/or revision of the previous mastopexy may be necessary to improve symmetry as well. Of course, much will depend on your physical examination and specific goals. Given your questions and concerns, you'll be best off spending additional time with your plastic surgeon communicating your goals carefully. Generally speaking, the best online advice I can give to ladies who are considering revisionary breast augmentation surgery ( regarding breast implant size/profile selection) is:1. Concentrate on choosing your plastic surgeon carefully. Concentrate on appropriate training, certification, and the ability of the plastic surgeon to achieve the results you are looking for. Ask to see lots of examples of his/her work.2. Have a full discussion and communication regarding your desired goals with your plastic surgeon. This communication will be critical in determining breast implant size/type/profile will most likely help achieve your goals. In my practice, the use of photographs of “goal” pictures (and breasts that are too big or too small) is very helpful. I have found that the use of words such as “natural” or "C or D cup” etc means different things to different people and therefore prove unhelpful. Also, as you know, cup size varies depending on him who makes the bra; therefore, discussing desired cup size may also be inaccurate. The use of computer imaging technology may also be very helpful during this communication process.3. Once you feel you have communicated your goals clearly, allow your plastic surgeon to use his/her years of experience/judgment to choose the breast implant size/profile that will best meet your goals. Again, in my practice, this decision is usually made during surgery, with the help of temporary sizers which allow for the most accurate evaluation of breast size/symmetry ( with you in the upright position).I hope this (and the attached link, devoted to revisionary breast surgery) helps.

Answer: Breast sagging. Hello! After evaluating your pictures i can see that you have a noticeable asymmetry cause by the implant im part but mostly by excess of skin that causes that sagging.I strongly suggest that you focus on a Mastopexy, or breast lift with implants, is a procedure designed to improve the appearance of sagging or ptotic breasts. You can use the same size(Not the same implant) that you have(340cc) or a little bigger and get some excellent results.Remember that a big implant will be firm at first but in long term will produce a more saggy breast. You take care
