The possibility of a photosensitive reaction with Spironolactone is approximately  .12%. (very low) I would recommend doing a test in a non visible area first. Should this be negative, the chances are that you will not react. Starting and stopping spironolactone has risks and should be avoided ...

IPL, (intense pulsed LIGHT), can activate herpes simplex. I would recommend pre-treating for 2 days with an antiviral and continuing the medication for a least three days. In my opinion, the intense pulsed light should do well for the capillaries. Depressed scars usually do not respond to this...

This needs an in-person evaluation to determine the appropriate strength of treatment. Keep in mind, it may take a series of treatments. I always combine IPL with microneedling/prp to improve outcomes. I recommend getting a formal evaluation with a cosmetic dermatologist. Best, Dr. Emer
