Well, after reading the response by Dr. Smoot, I see that when relaxed there does not appear to be a difference, but if you are concerned about remaining fat there is the potential for Liposuction. This procedure is a common resolution treatment for gynecomastia that is
primarily the result of excessive fatty tissue. During this procedure a thin
hallow tube called a cannula is inserted through small incisions and moved back
and forth as it loosens the fat and removes it through a vacuum suction. However, this is not necessarily the only option, and you should always discuss your concerns about results and uncomfortable finality with your surgeon. Best of luck to you!

I am sorry to hear of your disappointment with your gynecomastia surgery.  It appears that you only have deformity when you flex your pectoral muscles if I see your photos correctly.  When relaxed you appear fairly normal.  I would not recommend fat flaps because it is difficult to do and it may cause other problems.  The only options you have is to fat graft or remove some of the excess skin.  Fat grafting may not be easy since you are fairly thin and finding enough fat may be difficult. Either option could be done under local or sedation but I would have clear discussion with your PS as to what you want to accomplish.

Your best option to repair your chest is fat grafting which can be done in the office setting with minimal oral sedation; this will run you approx. 2K per session; you cannot do a fat flap because:1)you will compromise your areola incision2)you will be stealing from another area in your chest to correct your concern; this will leave you with a divot from where the fat was harvested.
