When I smile, my chin seems shapeless and it has a sag to the neck and also the fat behind the chin is very prominent. I can feel a sort of tendon or muscle going from my chin down the front of my neck that actually sticks out when I smile. Is it possible to remove or relax this? What can be done to eliminate the double chin and also give it a more tapered appearance? I'm in my 40s, nonsmoker.

As best I can tell from your photos you have a very underdeveloped mentum ( chin ). Most likely it can be easily corrected with a properly chosen chin implant. There are all styles, shapes, and sizes available to choose the right one for your needs. I do chin implants in my certified office...
Sliding genioplasty can be performed before or after. It can also be combined at the time of the other surgery. There isn't a clear consensus on whether before or after is better so you should discuss your expectations with your surgeon in order to decide what would be best.
