Need TT revision for indentations/abnormal contour. What type of revision would you recommend? (Photo)

11/21 Had ETT, lipo to flanks and back. 54 yo, 5'4", 167 lbs. Current weight 153 lbs. Wore garment as instructed with lipo foam boards 24/7 except for showers for 6 weeks. Indentations noted just a few days post-op. Had lymphatic massages but indentations progressed and now at 7 months post-op have an abnormal contour of waist/hips. Surgeon suggests revision with lipo to hips. What type of revision would you recommend? Do I need fat grafting to fill in the indentations?

Very likely, based on the photos and short of in person evaluation, that combination of selective liposuction (flanks, ends of incision), non-aspiration liposuction (dimples and lumps), possibly selective fat grafting, would be an option. Hope this helps. Good luck.

Indentations were already there before surgery, and after tightening the skin they only appear more obviously. Little fat grafting will improve the indented little areas. 
You have bid dog ears both sides and you are candidate for dog ear removal through an extension of the incision about 2 inches each side. Liposuction to abdomen, waist and lower back would be also beneficial to achieve a better result.

Liposuction of the hip areas will probably help improve your contour, but some may just be structural with your bones. Fat could be placed in the small indentations if you want.  You did get a nice result.

Thanks for the questions and posted series of photos. I agree with you in general terms. I would recommend bilateral large dog ear excisions with liposuction and fat grafting superiorly to indented areas. I would also use ultrasound guided fat injections.
