I'm ~2 mo post op FTM DI bilateral mastectomy no nip graft. I work out vigorously (rock climb, run, gymnastics, calisthenics). I have a lumpy mass on my left side underneath and extending next to my left nipple. It's hard, tender and uncomfortable. Should I be concerned? Also both sides still have a saggy almost fatty look and seem loose. Is this likely swelling or grounds for revision? It seems as if either not enough glandular tissue or skin was removed. Will they likely continue to firm up?

Answer: FTM TOP Surgery Hi labu1785,Thanks for the post and photos. You will likely require a revision surgery with removal; of excess skin and tissue as well as free nipple grafts. In an effort to preserve blood flow to the niplpes/areolas, too much tissue was left in place and not enough skin tightening was performed. Answer: Loose skin You still likely have a minor amount of swelling and it is definitely possible that over time you will get more skin contraction, however looking at your pictures, it appears that you have excess skin and the scars appear low. I would expect that at some time in the future, you will require a skin excision with free nipple grafting. Mastectomy without adequate skin removal often leads to this appearance. I would recommend waiting 6 months to a year before thinking about any revision surgery.
