im a 60yo female interested in a breast lift and possible implants . Is it possible to get a lift where the inframammary crease is visible . all the pictures i see it just looks like the nipple is moved is up and the breast tissue isnt actually lifted . this is not the aesthetic im looking for. Is it feasible to achieve the look i want at my age?

Answer: Breasts lift Dear Virginian, Thanks for posting your picture. From observing your picture Your breasts appear C cup with ptosis (droopiness) level 3. Also , the right nipple is closer to the midline than the left one and that can be corrected in order to achieve symmetry. I have been in practice for 38 years and what I have noticed through the years is that women at your age do not like large breasts. A fair number of my patients who got large breasts in their youth , request reduction at older age. so, I would think that just a full lift (key hole) will satisfy you. However , if you would like larger breasts that can be done with implants. The full lift procedure gives you nice projection, fuller upper pole and tighter lower pole that exposes the sub mammary crease. Always, consult with experienced board certified plastic surgeons who operate in accredited surgery center for your safety. Most importantly, check the before and after pictures in the photo gallery, to make sure that they are numerous and follow the above mentioned description plus nice scars. Answer: Maintaining full lift level unlikely. Thank you for your photo. You have nearly a Grade 3 ptosis. A lift ( with or without an augmentation), will position your nipple/areolar to the correct level and can initially tighten the skin of the lower breast enough so all the breast tissue is above the inframammary crease. However, the effects of gravity and the loss of tissue strength and elasticity at age 60 will very likely cause the breasts to settle some and cover the inframammary fold to some degree. The addition of an implant, which will add additional weight, makes the settling more likely. One possible option to increase the strength of your native tissues so that they support the weight of your breast better would be to add some form of a tissue scaffolding, which is implanted at the time of the lift, then incorporates into your tissues and leaves the tissues stronger than they were to begin with. You can discuss this in detail with your plastic surgeon. Best wishes.
