Muscles can pull the nostrils into a flared appearance, but cartilage and skin have elasticity and springiness that return the nostrils to their baseline shape when the muscles are relaxed. A hanging columella in itself is quite easy to fix. If that is all that is needed, it can be done under local anesthesia in less than an hour. Most commonly, I see this in patients who have had a previous rhinoplasty. I usually charge about $2000 for it.
