Fixing Under Eye Wrinkles with Blepharoplasty? (photos)

From your photos, it does not seems as though a blepharoplasty would be the answer. You do have significant hollows under your eyes, which is caused by a lack of volume. In someone your age, the best way to treat this concern is most likely with hyaluronic acid dermal fillers such as Restylane. The presence of hollows, dark circles and crinkles under the eyes can be quite distressing.
Not all that long ago, these problems were very difficulty to treat and surgery was the only option. However, surgical interventions to stretch the skin in the area, which were good for fine wrinkles and crinkles, often achieved the opposite effect by accentuating the hollows and shadowing.

Some fairly recent medical revelations have led to a radical shift in our approach. Physicians discovered that the hollows resulted largely from loss of fat volume in the area below the eyes due to shrinkage and gravitational sinking, or, as in this case, genetic or constitutional factors. They also discovered that either in large or part measure,  dark circles resulted from the fact that after the loss of the fat pad, the thin, delicate skin of the lower lid began to reflect the dark color of the small blood vessels and the muscles of facial expression immediately beneath it. These findings led to nonsurgical under eye rejuvenation.

By instilling a filler (I Belotero Balance, since it flows smoothly under the thin eyelid skin and has little risk of causing a bluish discoloration of the skin known as the Tyndal Effect), you can restore the lost volume to raise the hollow and also brighten the darkness by placing the filler between the dark structures below and the skin. Combining a tiny amount of Radiesse, a whitish colored injectable, with the Belotero can yield additional lightening, if desired.

The technique usually takes no more than five minutes per lower eyelid and is relatively painless, since the filler is combined with a local anesthetic. The procedure is one of a number of nonsurgical techniques that usually elicits a "Wow!" from patients the minute they see themselves in the mirror immediately afterward.

When crinkles are present, particularly in a younger person where skin elasticity is generally adequarte, instilling a microdroplet of a neuromodulator, such as Botox, Dysport or Xeomin, can be quite helpfull for smoothing this region. 

In the photograph it appears as though there is a sizable volume deficiency around the orbit. To start with, fillers can be used to test the aesthetic benefit.

Treating the under-eyes with filler such as Restylane or Belotero can help soften eye bags and hollow under eyes. For over-active muscles, lines can be softened with Botox. For older patients, wrinkles are best softened by fractionated laser treatments.
 

Consider lower eyelid fillers, which are safe, effective and natural. Lower blepharoplasty is not for you at this time. See an oculoplastic surgeon for evaluation.

A lower lid Blepharoplasty at age 21, and based on the anatomy shown, is an inappropriate procedure.  The photos show some degree of scleral show, and suggest a flat midface.  This combination is a relative contraindication for a subciliary blepharoplasty. Correction of the tear trough area can be resolved to some degree with Restylane or perhaps on a more permanent basis with a tear trough/ cheek implant.  Although a resurfacing procedure (fractionated co2 laser or a TCA peel) will help with the under eye wrinkles, they also carry the risk of pigmentary anomalies given your skin type.

You are very young to consider blepharoplasty, and you may want to first consider fillers. The hollows under your eyes could probably be improved nicely with a filler like Restylane or Belotero. The nice thing about fillers in this area is that they tend to last a long time (over a year). Another option is fat augmentation to the lower lids. Fat can be harvested from your abdomen or thighs and used to soften the lower lid bony rim. I think once you restore the volume, the wrinkles will improve.

You should undergo removal of excess skin from the lower lids via a skin only blepharoplasty. I don't think you have enough fat to roll over into the naso jugal groove. The naso jugal groove is very difficult to correct and I will only use fat or Juvederm. You might also benefit from a later laser procedure for wrinkles.

Your problem certainly has a genetic component: you have fine lines/wrinkles along your eyelids and a prominent "tear trough" which is really the outline of your eye socket [orbital groove]. You have deep set eyes, with not enough volume.  If you had more soft tissue volume in this region, this bony groove would not be as visible.

Your options for the fine lines [wrinkles] would be chemical peels or laser resurfacing. As you have a slightly darker skin pigment, if you chose laser resurfacing, I would recommend pre-treatment with skin bleaching agent [hydroquinone] for a few weeks prior to the laser and a few weeks after the laser to minimize hyper or hypo-pigmenation risks.

Your hollowness can be improved with either fat grafting or injectible fillers. In your case, I believe the injectible fillers are less risky, and more easily reversible if you are not happy with the results.

You are very hollow under your eyes and would benefit from volume as the other surgeons have identified.  In addition your upper orbit is also hollow and adding volume would significantly improve the overall appearance of your eyes.  Most importantly, a lower blepharoplasty with a sub ciliary incision has a very high probability of resulting in lower lid malposition in you and should be avoided.
