Which filler is recommended for nasolabial folds?

Question

I want to get filler in my nasolabial folds. I've been told I can choose between Voluma, Perlane and Modulas and that they actually inject in the cheeks and it 'pulls' up the folds, rather than injecting the folds themselves. Does that mean Voluma is best? Which will last longer and which will have fewer side effects such as pain and swelling? Thanks in advance.

Voluma is injected into the cheek area to address volume loss and can add a lift to the cheek area and soften up the nasolabial fold area with that lift. Voluma can last up to 2 years, but may require multiple syringes. Nasolabial folds can also be injected directly with other fillers such as Juvederm, Restylane, Perlane, or Radiesse. It is best to have a consultation with your physician to determine which is the best route to take to address your concerns and goals.

I recommend fillers to soften the nasolabial folds themselves and also lift around the mouth and cheeks to build underlying anatomical support. In our practice we combine Sculptra with other fillers "layered" on top and lasers, skin tightening treatments such as Thermage, Fractora, and Renuvion (J Plasma), thread lifting,  and peels to get the most conservative, natural look. Patients can be injected off label with a variety of fillers including Voluma, Perlane (now called Restylane Lift) and other fillers.  The key factors in choice of filler are:-depth of nasolabial fold-how thick or thin skin is- overall volume of cheek (determine if fillers in other location may be necessary)- desired stiffness of filler in smile lines (some patients don't like overly stiff smile lines and like a softer filler)

Voluma is one of my favorite hyaluronic-acid-based dermal filler for the cheek and midface area as it has a very natural look and feel. Voluma injected in such a way does create a slight lift to the face, which may be sufficient to smooth out your nasolabial folds. However, if they are deep you may also need an HA filler to soften those creases, such as Perlane.
Voluma can last up to 24 months, which is longer than most other fillers other than Sculptra. You probably will have some injection-related swelling, soreness and bruising that will last a few days to a couple of weeks from any dermal filler. Please consult a board-certified dermatologist who has experience sculpting the face with dermal fillers to find out your best options.

Voluma can improve the nasolabial folds in individuals that would benefit from more volume in the cheek area.  It is FDA approved to last 2 years.  If you have plenty of volume in the cheeks and it is just a nasolabial issue then a filler like Juviderm can correct the issue.  It lasts about 8 to 9 months.  In people with very deep nasolabial folds and flatness in the cheek area sometimes both are used.  They are equivalent in discomfort on injection because both contain local anesthetic in the product.  There is usually minimal swelling.

Voluma is the only FDA approved hyaluronic acid for the malar area or cheeks.  As we age, volume is decreased in the skin, muscle and bone structures.  The skin then becomes lax and the nasal labial forms seem to become deeper.  By injecting in the malar area (cheeks), one is correcting the problem in an anatomically correct area (where the volume was lost in the first place).  I do not recommend adding volume directly to the nasal labial folds.Thank you

For the treatment of moderate to severe nasolabial folds (smile lines), I like to use a dual approach with Voluma and Juvederm Ultraplus.  Voluma is placed along the cheekbone which will slightly lift the skin forming the nasolabial folds, and then I inject Juvederm Ultraplus along the nasolabial fold to soften this area further.  This dual approach slows down the progression of this fold and restores loss volume to the mid-face.

There are a few good choices to treat the nasolabial folds. Restylane, Perlane, Juvaderm, Radiese are good choices. Voluma will soften the nasolabial fold when injected to augment the cheeks and supposedly lasts for two years , whole the other choices last 6 to 12 months.

The nasolabial folds is where all the FDA trials for filler have been performed except for Voluma.  Having said that, Voluma is fine for this area.  The longest lasting filler will be Artefill.  Radiesse will probably last the second longest.  Injecting the cheek in important to help lift the folds, and in this area, a filler with a higher G Prime is recommended.
