I am told that a "youthful" face is like an inverted egg or triangle. If this is true, then wouldn't having fat or fillers placed along the jaw line and/or fillers placed under the nasolabial folds create too much volume in the lower face? I have studied many before/after photos, and most of time it appears as though the face takes on too much of a rectangular appearance after such procedures. Would a deep plane facelift be a better alternative?

Answer: Fillers to the Lower Face You are exactly right. A youthful face is egg or triangular shaped and smaller at the bottom. When fillers are used to get rid of lines and folds in the lower face, they do, indeed, make the face look more boxy and, to some extent, less youthful. However, their usefulness is in disguising the early signs of aging in someone who is not ready for or does not desire surgical correction of these problems. They have essentially allowed us to do things for a few years to keep you looking relatively youthful before you need surgery. Overall, 60% of the facelifts I do our repeats. Most of the first facelifts in these patients were done relatively early before the days of significant use of fillers. Now that we have fillers available, I do not know how many people will come back for a second facelift, or, if they do, when they will do so. Fillers also appear to be increasing the time between facelifts.
