I am an 18-year-old patient seeking advice on under-eye bunching that occurs strictly when I smile, as the area remain flat at rest. Could this tissue bunching be a structural issue anegative orbital vector and a lack of maxillary support? I would like to know if the underlying bone structure is causing the orbicularis oculi muscle and overlying skin to crowd upward when contracting, and whether conservative options like micro-focused neuromodulators or structural midface support would be the safest approach for someone my age, I am looking for a permanent option I’m not Into temporary procedures .

Based on the photos, the under-eye bunching appears to be dynamic and related to normal orbicularis muscle contraction when smiling rather than a condition that clearly requires surgery. At 18 years old, I do not see an obvious indication for permanent surgical correction. While facial structure and orbital support can influence the appearance, the changes shown are common and often considered a normal anatomical variation. Surgical intervention may not provide a predictable improvement and could create new concerns.
