Some insurance companies will approve you just with a letter from your plastic surgeon plus photos. Some insurance companies want you to have tried physical therapy, massages, chiropractic treatment, and pain meds and then your plastic surgeon writes a letter and submits your photos and they'll approve it. Some insurance companies may approve you but the reimbursement is so little that no plastic surgeon will accept it. I recommend you look for in-network plastic surgeons and call them to see if they accept insurance for breast reduction. If they do, see them for consultation and they know what to do from there. I have found that in different states insurance companies can be more lenient or more stringent in paying for breast reduction.
