I am 50 years old getting a lower facelift and want to address the lower eyes . I got one doctor saying a lower blethoplasty with fat reposition and another recommending periorbital fat injections. I’m worried about fat injections but I don’t see where a blethoplasty would works because I don’t see fat pads to reposition on me. at the same time. What would be your recommendations

The lateral portion of your upper eyelid incision has fullness. This needs to be corrected by extending the incision and connecting the skin.

The injury has likely caused bruising to the lower eyelids. Even though the trauma was to your forehead, blood pools in the lower lids due to the effects of gravity.
This should fully resolved. Continue with your usual skin care regime.
This is a difficult procedure.
Attempting to get significant skin lengthening will be at the cost of tightening skin in a perpendicular direction.
