Rarely, people may experience chronic inflammation from permanent sutures. This would be very uncommon. Everyone's face is the most swollen in the morning, and somebody's nasal appearance can vary depending upon the amount of water that they are retaining, especially after rhinoplasty. Swelling after a primary rhinoplasty typically lasts up to 1 year, with the tip of the nose taking the longest to settle. Noses with thicker skin also tend to experience more swelling. However, swelling 3 years after a primary rhinoplasty is uncommon. It’s possible that scar tissue may have developed, giving the appearance of swelling, though scar tissue wouldn’t fluctuate based on the day or temperature.
I recommend scheduling a consultation and taking pictures of your nose throughout the week to show your doctor the changes you’re noticing. If it’s scar tissue, it can be excised. If, for some atypical reason, you’re still experiencing persistent swelling, there are less invasive treatment options available as well.

At three years after surgery, you no longer have swelling related to surgery or healing.
The initially inflammation from the healing process has subsided long ago.
Fluctuations you’re currently experiencing are not related to healing from surgery. What you have now is your final outcome.
Lots of variables can affect the amount fluid in our tissues.
This includes sodium intake and other variables.
