I have already had a BBL. I am 29, 5ft and weigh 65kg. I have already had a BBL in September 2020. I would like a very small waist, much wider hips and large buttocks. One surgeon has recommended that I would need two more rounds of BBL to get the my desired look (see last photo). Buttock implants were not advisable as I was told they would not achieve my desired look. Please could anyone recommend further what I would need and if it is possible? Do I need to gain more weight as well? Thank you for any advice.
Answer: Wanting a small waist, wider hips, and very large buttock Let me start off by saying, whoever told you that “buttock implants were not advisable to achieve your desire look” was simply wrong and probably has never done buttock implants plants and/or is not skilled at doing them. On the other hand, you are in good company with many others who had high hopes for BBL but instead received disappointing results (lumpiness, unevenness, lack of projection in the central and/or lower buttock, post-deflation sagging, etc.). It is not quite as much the quality of the doctor but rather the quality of the procedure that is a large part of the problem. Thus, going back for round 2 BBL most often produces the same disappointing results but at another financial loss. Buttock (and cosmetic hip implants) indeed provide a more reliable, predictable, relatively safe, and permanent result, especially for adding projection to the buttock (and hips) and often smoothing out lumpiness. Please keep in mind that an extremely few number of board-certified plastic surgeons specialize in buttock (and/or hip) implants so be sure to consult with one that is reputable and experienced. Unfortunately you may have to travel long and far away from home to find this particular surgeon. Glad to help.

Answer: Hyper feminizing You should include proper before and after pictures so we can make an assessment of how your first procedure was done. I’m guessing your surgeon may not have put a strong emphasis on grafting your hips and outer thighs. That can take care of the hip part. It may be difficult to bring in your waist but weight loss is probably your best option. Reducing visceral fat content will help bring in the midsection. I’m guessing you’ve had liposuction done in that area as part of your first BBL so there probably isn’t much room for more improvement from liposuction though further fat transfer would require more liposuction and an attempt at bringing in your waist further should be done. For those who are candidates for a tummy tuck doing aggressive muscle tightening can bring in the waste to some degree. I would not recommend having a tummy tuck for the sole purpose of muscle tightening on someone who doesn’t need a tummy tuck. At some point each individual person needs to come to terms with who they are and the body they were born with. Best, Mats Hagstrom MD
