How do you fix wide lateral walls?

I’ve had a revision rhinoplasty and i am not happy with the results. My lateral walls are extremely wide now one side is bowing out and the lateral walls are almost under my eye and it’s as wide if not wider that my tip. The surgeon placed a dorsal onlay graft on my bridge which is not quite straight and is showing at the radix area. I have spreader grafts too. Would osteotomies fix the wide lateral walls. I do not want to reduce bridge height so im worried the osteotomies would do this.

That said, it sounds as though your rhinoplasty surgery was somewhat complicated with an onlay graft and spreader grafts. Spreader grafts inherently lead to widening of the middle one third of the nose.He's considered re-asking your question with photographs for a more definitive response from the experts. Thank you. Hello! Thank you for your question.Unfortunately without photos it is very hard to make an evaluation. But from my experience, most of the time we use rib cartilage reconstruction (and sometimes external graft) to reshape the tip and for the lateral walls you may need an alar base reduction. But as I told you without your face photos it is not possible to make an appropriate evaluation.
