Fat repositioning of fat removal to make my under eye a smooth transition?

Based on your photos you have a tear trough deformity and prominent nasojugal groove. Lower eyelid surgery with fat re-distribution is a well accepted technique for addressing both issues.In patients with tear trough deformity without obvious skin excess or underlying muscle laxity, both fat re-distribution (through a lower lid blepharoplasty procedure) and fat grafting are acceptable treatments. In patients with marked skin excess and /or muscle laxity, it is thought that fat re-distribution produces superior results than autologous fat grafting.

Common reasons for lower blepharoplasty include tear trough deformity or pseudoherniation of the orbital fat. An in-person exam would provide a more accurate diagnosis. Transconjunctival lower blepharoplasty with fat repositioning will provide nice results and a better lid-cheek transition in this case. Based on these photos it is very likely that transconjuctival lower blepharoplasty with or without fat flap transposition would work for you. This is decision that you should leave to your surgeon. Good luck.
