Chest wall deformity - is this a surgeon error?

I had implant surgery to correct what I was told was minor asymmetry, so was suggested to use same implants. Taking photos as the healing process was happening it became notable that my chest wall protrudes on one side. My asymmetry is now ten fold more in heights and volume and one nipple is lateral. Is this a surgeon error? Would it have been noticeable in theatre before the implants were placed? Extremely distressed and can't wear normal clothing at all now and I don't have more finances
