Based on the photos, this is a child and Botox would not be recommended. If this lump is not bothersome, it may be best to wait until he/she is old enough to independently decide they want to have surgery. Either way, it's best to seek a formal evaluation for a more definitive diagnosis and consideration of treatment.
I would not recommend Botox to smooth this area out. In addition, if this is a small child, I would recommend that you wait until he/she is of more adult size prior to doing anything surgical unless there is a specific mass that you can feel. If there is a mass and there are concerns, then surgery would be a much better option.
It is my understanding the patient is a child. In order to answer your question, some additional questions should be answered: when did the lesion first appear or was noticed? How does it behave - does it bother, changes in form, size and color? any discharge etc.? If answers are negative and the issue is of cosmetic nature, I would advise not to rush with surgery and rather observe it until pre-adolescent age. Getting second opinion wouldn't be a bad idea as well. Good luck
