Hello,Thank you for your question. I am sorry to hear the surgery has failed to fully treat your obesity. This can be due to the initial sleeve being too large, and/or stretching over time. Surgical options for you now include a re-sleeve, gastric bypass, or duodenal switch. Consultation with a bariatric surgeon will help you determine the best treatment based on your experience with the first surgery, your anatomy (evaluated through radiographic imaging), and your health goals. Please do not give up and know that many patients, regardless of their experience with their initial surgery, find long term results with the second surgery.Best wishes,Dr. Moein
It is possible to revise a gastric sleeve if you are experiencing weight regain or other issues. Depending on your situation, there are a few options to consider. One option is a sleeve gastrectomy revision. This could involve reshaping the stomach to make it smaller again or addressing other anatomical changes that may have contributed to weight regain.Another potential option is gastric bypass. This surgery is more complex but tends to provide more significant weight loss results, making it a common choice for people who’ve had a sleeve but still struggle with weight loss. For those who need more significant weight loss, especially if they have metabolic issues like diabetes, the duodenal switch could be considered. It is a more invasive procedure but can lead to substantial weight loss. It’s essential to consult with your bariatric surgeon to determine which option would be the most suitable for you. They will be able to assess whether a revision to your gastric sleeve is feasible or if transitioning to a different procedure, like gastric bypass, would be more effective.
I would recommend endoscopic review of your stomach or digestive diagram first to better get an insight into your first round, to figure out if your stomach was not narrowed enough, however usually that is not case in most of my patients, therefor I would maybe recommend gastric bypass, of course, after a deep disclosure of your general health and examinations. What is best is to review your eating habits first and life style whether you effectively adhere to a well planned- nutrition plan that best fits into your metabolic condition, then see other options.
Surgery is never 100%, and patients can experience weight regain. You're not alone.Try contacting your Bariatric Center that performed the original procedure and see what their recommendations are. You will most likely need an evaluation that will look at behavioral issues as well as possible mechanical causes. An UGI Swallow X-ray will help determine appropriate options. Most surgeons offer revisions in what they are most comfortable. Be sure to evaluate the surgical options of (1)Re-sleeve, (2)Gastric Bypass, and (3)Duodenal Switch.
If you have gained the weight, the reason needs to be investigated. If you eating large volumes, then Re-Sleeve is an option. If you still have restrictions then changing to a more malabsoptive procedure like Gastric Bypass should be considered. If you still eating high calorie diet, then you need a more invasive procedure with higher complication rate like Bilio-pancreatic diversion.
The reasoning is simple here... if you can eat more the sleeve stretched. If it worked before it can work again by recalibrating it.I don’t agree with immediately wanting to convert to another more aggressive procedure with its own set of undesirable effects.OCC Sleeve Rescue... it works. Proven track record.
Hello,I would definitely consider the case for an evaluation, reading the information you have provided the gastric revision can be taken in consideration. The first thing is that you must evaluate yourself with a certified bariatric surgeon who can perform the Switch, also review your case and then proceed to take the best decision for you. Do not get discouraged in the search of options to improve your health. My Best Regards,
