Do Doctors detach skin from the muscle/tendon during dermal filler procedures on cheeks, and does this ever recover/reattach?

Whilst having cheek filler 9 months ago my Doctor mentioned that she was removing the skin from the muscle with a cannula/needle and this would only need to be done once as it does not attach itself again. This has really worried me since and I have noticed a drop/sag in my cheek appearance overall and have developed more prominent nasolabial folds. Why would a Doctor do such a drastic measure? Please advise.
