How to Tell Between Dermal or Epidermal Hyperpigmentation?

How do I know if I have dermal or epidermal hyperpigmentation. I think I have a dermal hyperpigmentation because I have been using a regiment of Hyydroquinone 4% and Tretinoin .05% with minimal improvement.

I usually advise my patients not to pick at their skin following a chemical peel but rather replace that compulsion with frequent application of bland moisturizers (e.g. aveeno, cetaphil, cerave).  You otherwise place yourself at increased risk of scarring.
