2nd opinion for tear trough filler & puffy fat pads? (Photos)

I was recently told I wasn't a good candidate for tear trough filler because my fat pads were too puffy. This was an injector and not an MD. They suggested I get an expert second opinion.

Although additional photos of the entire cheeks would have been helpful, there is a strong suggestion that volume deficit in the cheeks is contributing significantly here to the under eye issues, namely the bags, tear troughs and hollows. A filler cheek augmentation, properly performed, would very likely not only create a more heart-shaped, oval face, but secondarily improve the under eye area. You would be wise to consult with an experienced, board certified cosmetic dermatologist and be sure to ask to see his/her personal before and after photos before agreeing to proceed. Best of luck.

I would agree that you are not a good candidate because you have very puffy fat pads. These are probably genetic and tear trough treatment would only make them worse. I recommend a consult with a plastic surgeon for this area.

I don’t believe that you have is a typical tear trough. You also don’t have bags of the lower eyelids because when you touch them you probably feel bone. You should not have fillers placed into the lower eyelid area which appears to be quite hollow. You need to have fat grafting in that area in order to correct the deficiency. This condition is inherited most likely from your father side. With a fat grafting specialist, Plastic surgeon the results should be natural and permanent. How satisfied you will be with tear trough filler depends upon how much improvement you expect. I tell my patients  with these fatty deposits that they will get to second base but  it won't be a home run. If done well, you will get some improvement and if you don't want surgery it might be a good compromise. Even with surgery you might have hollowness that you are not happy with. In addition, if your cheeks need filler, which I can not see in these photos, you might get some improvement in way under eye area looks as well.
