What can be done to remove a small dark spot that developed after lip filler? 6 years post op (photos)

Based on your description and photo, the small dark spot you’re noticing may represent a few possibilities: it could be a mucous retention cyst (mucocele), a clogged salivary gland duct, a vascular lesion, or scar tissue related to prior filler or trauma from attempts at self-removal. Since it has persisted for years and hasn’t responded to dissolving filler, it’s less likely to be residual filler alone.

Treatment depends on the exact diagnosis. Options may include:

Minor surgical removal if it is a cyst or scar nodule.

Laser treatment if it has a vascular component.

Steroid injection or excision if it is inflammatory or scar-related.

I believe it's possible that this dark spot on your lip may not be related to lip augmentation 6 years ago. Most brown spots are benign, however, some can represent malignant melanoma. I suggest you have a few consultations with board-certified plastic surgeons in your community.
They looks like a blood blister, but that would be unusual this far after your treatment. Did you bruise after the treatment?
I’m sure local dermatologist would be happy to help as well.
I generally have more faith in plastic surgeons when it comes to sensitive surgical procedures unless the dermatologist does mohs reconstruction. Not that you need reconstruction. You just want to make sure removing the lesson doesn’t leave a bad scar or defect.
