Answer: Is this a caudal septal deviation? Most likely, yes. In some cases the caudal septum is fine but the medial crura of the tip cartilages are deviated. However, most often the caudal septum is the issue. This can be remedied with a caudal septoplasty and any other issues with the septum can be addressed at the same time. Hello waterleak,Thanks for your question.Yes, in my opinion, you seem to have a caudal septal deviation. An in-person exam would be necessary to confirm this. Your areas of concern can be improved upon with a septoplasty procedure and possible repositioning of the maxillary crest. As far as your columellar show is concerned, a little columellar septal suturing and possible anterior columellar shave can improve the roundness that you see on profile. A caudal septal deviation just means that the end of the septum that is visible is deviated. When the columella is hanging as presented in the photographs, a columella-plasty can be performed to reduce the amount of columellar show. That makes your nostril on that side smaller.A septorhinoplasty is the name of the surgery that can help straighten the caudal septum. The caudal portion of the septum supports the nasal tip, and hence is an important structural part of your nose. The caudal septum is straightened and tip support is reinforced during surgery.
