Most likely, yes. In some cases the caudal septum is fine but the medial crura of the tip cartilages are deviated. However, most often the caudal septum is the issue. Hello waterleak,Thanks for your question.Yes, in my opinion, you seem to have a caudal septal deviation. An in-person exam would be necessary to confirm this. Your areas of concern can be improved upon with a septoplasty procedure and possible repositioning of the maxillary crest. As far as your columellar show is concerned, a little columellar septal suturing and possible anterior columellar shave can improve the roundness that you see on profile. A septoplasty is performed to improve airflow dynamics through the nose by straightening it to improve air flow. A caudal septal deviation just means that the end of the septum that is visible is deviated. When the columella is hanging as presented in the photographs, a columella-plasty can be performed to reduce the amount of columellar show.
