Two years post facelift, neck lift and chin implant - how can this be fixed? (Photos)

I had a facelift, neck lift, and chin implant procedures done in August 2024. The implant was supposed to be a small implant that I believe that a larger implant was used. When I asked my surgeon at 6 month postop if I could see the actual implant that he had used so that I could see how big it was he said he didn't have any in the office that he could show me. I told bim that I didn't like how big it was and how my chin looked. He said to give it some time to heal. Two years later and it doesn't look better. Is it normal for my neck and implant to look like this 2 years postop?

At almost 2 years after a facelift, neck lift, and chin implant, the result is mature. If the chin still looks too large or unnatural to you, it is unlikely to improve significantly with more time alone.
A chin implant that looks too prominent can be due to the implant being too large, sitting too low, sitting too far forward, shifting position, or not matching your jaw and neck anatomy. The neck appearance may also be affected by scar tissue, platysma bands, skin laxity, submental fullness, or how the implant changes the chin-neck angle.
The first step is to identify exactly what implant was used and where it is sitting. You can request your operative report and implant record from the surgical facility. Imaging such as a CT scan can also show implant size, shape, and position.
Revision options may include exchanging the implant for a smaller one, repositioning it, removing it completely, or addressing scar tissue and neck contour separately. If the neck lift result is also part of the concern, a revision neck evaluation is needed, but it should not be done blindly without understanding the implant issue first.
My advice would be to see a revision facial plastic surgeon and bring your operative report if possible. Ask whether the problem is implant size, implant position, neck scar/tethering, platysma, or residual skin laxity. At this stage, improvement is possible, but the safest plan starts with a clear diagnosis rather than simply waiting longer.

Thank you for your question and photo. 
First off, I am sorry that you are going through this and I'm sure it can be frustrating. It is hard to tell exactly what is going on from that photo, but generally the chin implant should be sitting just on the anterior and lower surface of the chin bone. This area actually looks fine from what I can tell. What I notice more is the excess skin and dimpling around the chin. I cannot tell if this is from the incision or from another issue. Ultimately I think your best option is going to be discussing with your primary surgeon at this point vs getting a second opinion from a plastic surgeon that specializes in facial rejuvenation surgery.

Based on this photo, it would be difficult to say what is exactly happinining with your implant; in person evaluation is needed for more details. However, excessive scarring seems to be to be dominant cause of deformity of neck and chin. It is very likely that revision surgery which will include implant replacement and neck lift revision would be the way to deal with this deformity. I would suggest 1-2 second opinion consultations with a board certified plastic surgeons with good reputation in face/neck lifts. Good luck.
