I am a 55 year old female, 115 lbs, active with no health issues. I am having sagging skin around my mouth. I had Sculptra on the left side of my face about 9 months ago and typically have that every year and a half. I am not interested in having a lower face lift but want to know what I can do to help plump that sagging area.

Answer: Consider A Nonsurgical HA Filler Facelift For Treating Facial Sagging & Jowls A profile view would have been very helpful in here. However, at least from the front face view provided, it appears that a number of issues can be treated nonsurgically via the use of appropriately selected robust HA fillers properly injected. For example, volume deficit in the lateral portions of the mid-cheek can be corrected with the use of a robust HA volumizing filler. Properly injected by an experienced, board certified cosmetic dermatologist, this can create a more desirable, heart-shaped oval face, as well as improve secondarily both the under-eye tear troughs and diminish the smile lines. The use of an appropriate, robust HA contouring filler in the region of the posterior angle of the jawline and along the jawline would likely further diminish the sagging there and better define and straighten the jawline itself. And, finally, IF still necessary, a robust HA lifting filler can be used to straighten the downturned, marionette lines around the corners of the mouth I gave up using Sculptra many years ago because it was expensive, the results were not immediate, and treatment eequired about three separate treatment sessions in order to achieve results. It's original advantage was that the final result lasted longer than other injectable products at the time. But, subsequently, many more robust HA fillers were approved that offered immediate results after just one treatment and gave rise to quite prolonged durations of results. You would be wise to make sure to consult an experienced, board certified cosmetic dermatologist filler expert.Hope this helps and best of luck.

Answer: Jowling Hi! This is a great question! As we age, we lose collagen and support of the skin, and this usually results in skin laxity, wrinkles, and jowling. It is great that you have done sculptra! This would be recommended for you, but please ensure that you are having enough sculptra injected to help address your concerns. As a very general rule of thumb, about 5 vials of sculptra should be used for your concerns, considering your age. In addition, a comprehensive treatment plan of: forehead and neck botox, cheek and jawline fillers, perioral fillers, and collagen boosting treatments (RF microneedling) should be implemented to help thicken/tighten the skin.
