Is there a risk of micro fragments coming off around the screw overtime as I'd worry about silicone leaching into my body?

My silicone chin implant has drifted up ; I want it repositioned & anchored with a screw. [1] Is there a risk of micro fragments coming off around the screw overtime so I'd worry about silicone leaching into my body - real problem? [2] Can you point me to any medical articles supporting use of screws in silicone chin implants? [3] Can it be repositioned & screwed under local anesthesia? Thanks.
