I had a chin implant which I got 4 years ago removed 1 year ago and now have dynamic chin ptosis and lip incompetence. I was wondering what can be done about this, what type of doctor I should see (plastic surgeon vs OMFS), and if there is anyone in the Philadelphia area who could help me. I'm not interested in another chin implant. I would like my chin to stop retracting when I smile, to not have to purposefully force my mouth closed with my sagging upper lip, and decrease chin sagging at rest.

Hello. Unfortunately, you are in a very unfortunate situation and you do not have the luxury of saying that you do not want a chin implant. Because of the combination of a small jaw and these relaxed muscles, you have the current situation. A smaller implant than your old implant must be placed. Afterwards, the muscles here should be shortened and combined with witch jaw repair. Please review our similar surgery results. You need an experienced doctor on this subject. Good Morning! Thanks for the photos which are helpful to show what you're referring to. At times, to ensure no implant migration has occurred, I may recommend a CT Maxillofacial to see what's going on deeply. If the positioning seems good, then it could be trauma to the mentalis muscle. If not reapproximated, it can lead to a "witch's chin" deformity. This may require going back into the chin and trying to place the muscle into a proper, fixed position. As for which surgeon, a oral surgeon may be able to assist or a facial plastic surgeon. Hope this helps.
