What can be done about dynamic chin ptosis after implant removal 1 year ago? (Photos)

Question

I had a chin implant which I got 4 years ago removed 1 year ago and now have dynamic chin ptosis and lip incompetence. I was wondering what can be done about this, what type of doctor I should see (plastic surgeon vs OMFS), and if there is anyone in the Philadelphia area who could help me. I'm not interested in another chin implant. I would like my chin to stop retracting when I smile, to not have to purposefully force my mouth closed with my sagging upper lip, and decrease chin sagging at rest.

Good Morning! Thanks for the photos which are helpful to show what you're referring to. At times, to ensure no implant migration has occurred, I may recommend a CT Maxillofacial to see what's going on deeply. If the positioning seems good, then it could be trauma to the mentalis muscle. If not reapproximated, it can lead to a "witch's chin" deformity. This may require going back into the chin and trying to place the muscle into a proper, fixed position. As for which surgeon, a oral surgeon may be able to assist or a facial plastic surgeon. Hope this helps.
