Generally speaking, you want to reach your goal weight before you consider a surgery, otherwise your results will be suboptimal if you continue to lose weight after the surgery. Board certified PSs look for a between 25 and 30 when they perform aesthetic procedures so enhance results and to reduce the risks of complications during and after surgery. Thinner patients with a BMI above 24 are outstanding candidates in my opinion, because the fat transferred is extremely resistant. Results are largely dependent upon the skill of the surgeon, his/her ability to remove fat without creating contour deformities, and your expectations. You would need a consultation via email or Skype with a board certified PS to determine your available fat stores.
Fat transfers can be done on those in good health. If you have a high BMI and plan to try to loose weight, it is best to avoid the fat transfer since it will shrink with weight loss. If you have a high BMI and don't plan to loose weight, perhaps you should reconsider trying to loose weight to be in the best health you can.
There is no absolute cut off. It’s up to each individual plastic surgeon of what they feel is appropriate.It varies substantially case by case.
