Dysport complications - could it be permanent? Dents in forehead (Photos)

I received 40 units in the middle area of my forehead in November, everything was fine but then I went for check up a month later and my doctor convinced me to get extra 10 units above my brows, since I still had some movement in that area. After that I slowly started feeling changes in my brows, tingling and numbness around my eyes and temple area. My brows went completely straight no arch also my forehead got completely flat and stretched like 2 cm. Could it be permanent?
