Can a canthoplasty/pexy without upper eyelid blephoroplasty correct sclera show and negative canthal tilt? (photos)

I have lower eyelid retraction revealing scleral show and negative canthal tilt. My primary goal is to have my lower lid raised and for my tilt to go from negative to neutral I've attached pictures which show the issue. In the first 2 pics, my eyes are relaxed and as you can see, my eyes look downturned. In pics 3 and 4, I am squinting my eyes, and my eyes look a lot better that way. I have hooded eyes, but I don't want to do anything to make them not hooded.

It is hard to tell in the picture, but it also looks like you have some significant brow asymmetry which is contributing to the eyelid asymmetry. A blepharoplasty would certainly help some of the excess skin, but I would be concerned this would not address the overall asymmetry which likely...

The location and pattern of these holes seem to coincide with where your sutures were placed in your skin. Frankly, this is an unusual situation. Any treatment that would break up this scar, generally termed a "local tissue rearrangement," should help this area.
