Hello, I had a chin implant which a little bit improved my appereance, however I know I have more problems, my face is not just oblong, I see I have something wrong with my jawline, it points down. How could I improve this?? Please help me, I cannot look at myself anymore as I am feel very ugly :( I also had six month smile braces, but it did not improves jawline.

Answer: Sliding genioplasty or two-dimensional augmentation genioplasty You are a tuypical case of poor results with chin implants needing a revision to replace the chin implant with an osseous genioplasty; in your case to augment forward and probably (to be discussed) also augment vertically (with bone supply). It is true that 97% of surgeons use the implants instead of the osseous surgery to adjust the chin, but this is a matter just of lack of experience and comfort; implants play no role when there is an autologous alternative which, actually, has plenty of superior advantages. Being an option, chin implants have serious disadvantages, specially in the mid and long term, patients are rarely happy with them after 5-6 years, all are out by then or later. Consider the chin advancement by means of sliding genioplasty, the advantages are:-lifetime procedure, no more revision or renewal procedures-more natural than implants-no visibility of implant edges during gestures-improves the double chin effect at the sub-mental muscles (floor of mouth) hammock caused by lack of tension there; also improves the shape and tension of perioral muscles; implants do no offer this feature-horse-shoe enhancement, covering the chin and also the jaw sides of it-no future displacements or neuritis of mental nerves-practically zero infections and 100% healing of bones-no need for complex plates and screws that may need future removal; just steel wires synthesis may suffice, they stay buried into bone callus The cons are:-expensive, but cheap if you consider the lifetime pros-technically demanding, few surgeons are experienced or comfortable (though I personally perform 100% of the chin augmentations by this osseous method, always after offering the patients both options)-scary for the patient (though not so much if you explain well the stuff around it) You'll find in this website dozens of negative experiences with chin implants in the short and mid term, do read my profile's Q&A and you'll find them too. Answer: Jawline You have a steep jawline or a high mandibular plane angle of which the the small chin is just one symptom of it. The use of a chin implant may not have been the best chin augmentation approach since your chin needed to come much further forward and be shortened in height at the same time. This can only be done by a sliding genioplasty. Whether you would also benefit by vertical lengthening jaw angle implants requires a front view picture to assess.
