Vertical and horizontal projections with genioplasty or implant

Without an exam your question cannot be answered.  You will need to see an Oral/Maxillofacial surgeon to see if othognathic surgery, genioplasty or using an implant is best for your specific case. It should be noted that patients with severely weak jaws or abnormally functioning jaws may be better treated with other facial surgery (orthognathic surgery), although a chin augmentation may be an option at a later time. These patients should seek the advice of a plastic surgeon to determine which course is right for them. Thank you for your photos and your question. First of all I would like to discuss implant vs. genioplasty. If you are seeking and vertical improvement, this cannot be done with an implant in my opinion. Implants are good for creating a few mm of projection, but cannot address vertical height with a natural appearance. A sliding genioplasty will be able to address both the horizontal projection and vertical length in a natural way to balance out the rest of your facial features. During my consultation I perform a simple facial analysis that will determine how much projection and vertical length you will benefit from. Ideally the lower facial height is equal to or up to 3mm greater than the midface height. Often times a weak lower face will give the illusion of the nose being larger than it actually is. By balancing your chin, the appearance of your nose can also become less prominent. I wish you luck in determining if this procedure is right for you.
