This indeed is an unfortunate consequence of either high dose corticosteroid injections or the injection been given into the fat layer instead of more appropriately into the muscle layer. If the fat loss has finally stabilized then it is a good time to start reconstructing the defect. Probably your best option for this relatively small area would be fat grafting. Note that kt may take two or three sessions to get you back to about normal. Glad to be of help.
Similar to the responses, I think fat grafting is your best option. Although instead of just injecting fat into the defected area, there is a cannula that can be used to first separate and release the scar tissue from the underlying structures. It will allow the fat to survive better by making space instead of just injecting it under all pressure if the scarred area. I would be sure to choose a Board certified Plastic Surgeon who is familiar with fat grafting. I do multiple fat grafting procedures every week for my breast reconstruction patients. Good luck!
I'm sorry to hear about this issue. I do not think that it can be corrected completely, but a series of fat grafting may improve it significantly. He would likely need several rounds of fat grafting performed at different times. I would consider an in person visit with a board-certified plastic surgeon.

I am so sorry to see you with this result. I think this will be difficult to resolve, but could be improved a lot by having a series of fat grafts to replace the fat destroyed. I do think it will take being done at least 2-3 times to build fat back over time.Discuss with a plastic surgeon who does a lot of fat grafting work. Good luck.
