XL BA: Overfilled Saline Breast Implants vs Expander Implants

Expander implants are temporary devices used to stretch the breast to a larger size and then are subsequently replaced with a permanent implant. Saline breast implants are over filled a prescribed amount by the implant manufacturer to help reduce visible and palpable rippling of these implants.

Expanders are considered temporary devices, and in the US  the largest saline implants that are available are 800cc's.  It is not recommended to overfill them above the recommended fill volume.

Thank you for the question. Saline breast implants are usually filled within a range provided by the breast implant manufacturer. Utilizing saline volumes in the upper range of the manufacturer's recommendations helps cut down on concerns such as breast implant rippling. Sometimes, patients who are looking for a certain "round, fake look" benefit from the use of overfilled saline implants.  This involves filling the saline implants past the range provided by the manufacturer.  I have found, through significant experience, that for the appropriately selected patient, very nice outcomes can be achieved safely. You may find the attached link, dedicated to "larger" breast surgery concerns, helpful to you as you learn more. It really depends on what size you want to go to.I have specialized in XL breast implants for many years. Yes, you should be able to go bigger with an open capsultomy and if not enougfh room then placement of a tissue expander and then switching to a custom silicone breast implant. 
You can get a "standard" sized saline implant overfilled but I recommend to about 1500cc max. If you want to go larger,then I recommend and XL breast tissue expander which I can obtain for my patients in XL sizes. These start XL and go to larger sizes than you wish. These are filled through remote port(see how on my You Tube video) while the saline implant requires surgery for each fill and it woiuld take several to get you to a true XL size!!
