I've posted before and been told I'm a good candidate for these. But I have so many questions...what will these procedures look like years from now? Will it create more sagging? I have a very fat face. Will the fat sag without the supper of buccal fat and double chin? I only want a very little amount of buccal fat removed btw. I like the natural look and don't want to look contoured, but just have a slimmer face. Thank you!

Answer: What will buccal fat reduction and chin lipo look like after aging? Hello, thank you for your question. As we age, we naturally lose fat from our faces, and an aggressive treatment can result in a sullen, sunken result. Buccal fat removal or bichectomy is the plastic surgery procedure to permanently removed the fullness of the lower anterior cheeks. When performed correctly, it shouldn't give a sunken look, unless it is desired and requested by the patient. If somebody has good cheek bones, him or she shouldn't see a significant aging appearance of the face with aging after buccal fat removal.

The key is that the buccal fat pad is a persistent deep fat pad with higher stem cell ratio. This fat responds differently than the superficial fat pads (those are the one that make us all look older). The buccal fat pad is deep in the face. It has fascial attachments that are different!. For natural results we are able to actually sculpt the buccal fat pad that is there even. I have started calling this procedure buccal fat pad sculpting exactly because of this. The most dramatic effects come in the patients who need it the least, the already thinner type face. In the more round or fuller face the buccal fat pad contributes less to the overall facial fullness than the larger subcutaneous fat compartments. As a result buccal lipectomies make some difference but not as much as most patients would like. There is no evidence that buccal lipectomies contribute to facial skin sagging short or long term. You are more likely referring to the risk of creating a gaunt looking face long term, which is not a risk in the naturally rounder or fuller face.
