I had surgery done last year April so it’s been over a year. The first 6 months I didn’t notice it but since my swelling has gone down I’m left with a major deformity in my forehead. I’m very unhappy with it and I’m worried about having a revision because I still have numbness from my last surgery, I don’t want to make it worse. What are my best options

Forehead dents after surgery and what to do? I would need to see you in person as well as an operative report from your surgeon to determine what can be done to repair this. Make sure you see a very experienced FFS surgeon to repair this.

You didn't mention what surgery you had, but I would guess it was a browlift with muscle removal of the corrugator muscles. Unfortunately, this is a predictable outcome of removing those muscles.  The best answer will be to recreate some volume in the area. It can be done with injectable fillers (though this is a most risky area for fillers), or, an implant.
