Our treatments use combination therapy to improve the epidermis and dermis for most deep scars. The options available for acne scars depend on the character of your scars, in terms of topography and contour. We see atrophic and hypertrophic acne scars, as well as scars that have hypo and hyperpigmentation. We begin each scar evaluation with an assessment of scar quality and subtype. Our main concern is rebuilding collagen and that is why we use a HIDEF protocol that includes combination therapy. Morpheus 8, scar subcision, fibrous release, fractional laser, CO2 laser, Thulium laser, PDL Vbeam laser, and fractional resurfacing can be used in addition to TCA Cross and chemical peels to further improve skin. Patients should start Melarase AM and Melarase PM for active hyperpigmentation. Always begin with a consultation to decide whether early subcision should be started. Chemical Peels are a great way to evening out skin tone, lightening hyper-pigmentation and reducing pore size. A great one with these effects is the VI Peel with a double booster. This peel will have your skin peeling for about 7-10 days, exfoliating the dead skin cells and some of the hyper-pigmentation. With each peel your skin will lighten up to 20%. In addition to the peel we recommend using a bleaching and blending cream at home to aid in the process of removing the hyper pigmentation. Our favorite is ZO Melamin and ZO Melamix that have a 4% HQ in them. Whenever you bleach you need to blend the skin as well! After bleaching and blending you will want to finish with a Retin-A Cream to continue exfoliating at home, we love ZO Retamax! Peels would work extremely well with this case and definitely help even out the pigmentation. I would add a bleaching cream such as a hydroquinone based product and Retin-A to this regimen. Please consult an expert in chemical peels. Best, Dr. Green
Dear fatima418523: There are many peels that can improve hyperpigmentation on the face. In my office, we do and have used peels with salicylic acid, glycolic acid, tretinoin, and many other agents. In my experience, the most effective is the ViPeel with booster. Using the kit afterwards improves results. You will likely need to wear a sunscreen daily or an antioxidant to prevent the pigmentation from returning. All the best, Dr. Clark
